Drop page 5; the A4 print is the review copy

How the conductor reads it (not his words)

• PQ5: in the A4 edition only, 00b's screenshot of the companion website, which sat alone on page 5, is left out (a4.css). The A4 build is now 106 pages with one short page left, the further-reading page.
• RC1-A4, the purpose: the A4 copies are review copies, to be read and to bring advance buyers to Leanpub and Amazon, not copies for sale. Put to him: an "Advance review copy" mark on the A4 cover, and a line saying where to buy.
